1. The Clinical Spectrum: Decoding Liver Disease Terminology
Chronic liver disease is not a static condition. It is a clinical continuum that can progress from fat accumulation to severe, life-threatening organ failure. Understanding this spectrum can help patients participate more actively in their clinical care.
Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD): This is the updated clinical term for what was previously known as Nonalcoholic Fatty Liver Disease (NAFLD). It indicates accumulation of fat in the liver, or steatosis, together with at least one cardiometabolic risk factor, such as obesity, type 2 diabetes, high cholesterol, or high blood pressure.
Metabolic Dysfunction-Associated Steatohepatitis (MASH): Formerly known as NASH, this is a more severe stage in which liver fat accumulation is accompanied by active inflammation and liver-cell injury. MASH can drive progressive scarring.
Hepatic Fibrosis: Fibrosis is a biological wound-healing response to chronic liver injury. Persistent inflammation can cause specialized liver cells to deposit excess collagen, forming scar tissue. Fibrosis is graded from F0, meaning no fibrosis, to F4, meaning cirrhosis. Importantly, fibrosis is not necessarily permanent; addressing the underlying cause may allow some regression of scar tissue.
Cirrhosis — Compensated vs. Decompensated: Cirrhosis represents advanced fibrosis, or F4, in which extensive scar tissue distorts normal liver architecture. In compensated cirrhosis, the liver is significantly scarred but can still perform essential functions without major clinical complications. In decompensated cirrhosis, complications may develop, including fluid accumulation in the abdomen (ascites), hepatic encephalopathy, or gastrointestinal bleeding.
Sarcopenia: Sarcopenia is severe loss of skeletal muscle mass and function. In advanced cirrhosis, it is an important clinical complication associated with poorer outcomes.
2. Nutritional Therapy: Weight Management and Diet Quality
Medical Nutrition Therapy (MNT) is an important component of liver-disease management, but nutritional priorities change substantially once cirrhosis develops.
For non-cirrhotic metabolic liver disease, treatment may focus on improving metabolic health and achieving gradual weight reduction when overweight or obesity is present.
For people with cirrhosis, preventing malnutrition and loss of skeletal muscle becomes a major priority.
Weight Loss and Liver Health
In people with overweight or obesity, evidence shows a dose-responsive relationship between weight loss and improvement in liver disease.
At least 5% weight loss: Can produce a clinically meaningful reduction in liver fat, or steatosis.
Approximately 7–10% weight loss: Is associated with a greater likelihood of improving steatohepatitis and related liver injury.
10% or greater weight loss: Has been associated with the greatest likelihood of histological improvement, including possible regression of fibrosis in some patients.
Weight-loss targets should always be individualized according to the patient's disease stage, nutritional status, medications, medical conditions, and clinical supervision.
Mediterranean-Style Dietary Pattern
There is no single “magic” macronutrient ratio for liver health. A sustainable nutritional strategy and overall dietary quality are more important.
A Mediterranean-style eating pattern can emphasize vegetables, whole fruits, legumes, whole grains, nuts, extra-virgin olive oil, fish, and other minimally processed foods.
It is also reasonable to reduce saturated fats, added sugars, highly processed foods, and sugar-sweetened beverages.
Coffee: Observational evidence has associated moderate coffee consumption with lower odds of significant liver fibrosis. Coffee should be viewed as a possible supportive dietary habit rather than a treatment for liver disease.
Alcohol: In advanced fibrosis and cirrhosis, alcohol avoidance is an important clinical consideration and should be discussed with the treating medical team.
Supplements and “Liver Detoxes”: Patients with liver disease should be particularly cautious about unregulated herbal preparations and products marketed as liver detoxes. Supplements can interact with medications or, in some cases, contribute to liver injury. Discuss supplements with the healthcare team before using them.
3. The Important Nutrition Shift in Cirrhosis
Nutrition management changes considerably when cirrhosis develops.
The cirrhotic liver has reduced ability to store glycogen. During prolonged fasting, the body may therefore rely more heavily on alternative energy sources, including skeletal muscle protein. This contributes to the risk of sarcopenia.
For this reason, preventing prolonged fasting and maintaining adequate energy and protein intake become important therapeutic objectives.
Avoid unnecessary aggressive calorie restriction: Patients with cirrhosis should not automatically follow conventional weight-loss diets. Energy needs should be individually assessed.
Protein: Daily protein intake in the range of approximately 1.2–1.5 g/kg/day is commonly recommended for people with cirrhosis, depending on the clinical situation.
Protein should not routinely be restricted in patients with hepatic encephalopathy because inadequate protein intake can worsen muscle loss.
Frequent meals: Eating smaller meals throughout the day and avoiding long fasting periods may help maintain nutritional status.
Late-evening snack: A late-evening snack containing carbohydrate and protein can help shorten the overnight fasting period and may help reduce muscle breakdown.
4. Medical Management
Nutrition is only one component of chronic liver-disease treatment. Medical and pharmacological treatment becomes increasingly important as disease progresses.
Resmetirom
Resmetirom is a thyroid hormone receptor-beta agonist used for selected adults with non-cirrhotic MASH and moderate-to-advanced fibrosis.
Patients should ask their hepatologist whether they meet the appropriate clinical criteria and what benefits, risks, contraindications, and monitoring requirements apply.
Semaglutide
Semaglutide, a GLP-1 receptor agonist, may be appropriate for selected patients with metabolic disease, obesity, type 2 diabetes, and MASH depending on the clinical indication and current regulatory guidance.
Treatment decisions should be individualized by the prescribing clinician.
Ascites
Patients with cirrhosis who develop ascites may require diuretic therapy, commonly involving medications such as spironolactone and furosemide.
These medications require medical supervision and monitoring of kidney function, sodium, potassium, blood pressure, weight, and fluid status.
Hepatic Encephalopathy
Lactulose is commonly used to manage hepatic encephalopathy. Rifaximin may be added in selected patients, particularly when episodes recur.
These therapies require ongoing medical supervision.
Patients with cirrhosis and ascites should also discuss the safety of medications such as NSAIDs—including ibuprofen and naproxen—with their medical team because of the potential for kidney and other complications.
5. Practical Action Plan for MASLD and MASH
If you have uncomplicated MASLD or MASH, discuss these steps with your healthcare team:
✓ Determine your baseline fibrosis risk rather than relying only on ALT and AST liver enzymes. Ask whether your FIB-4 score should be calculated.
✓ If you have overweight or obesity, discuss an appropriate gradual and sustainable weight-loss target.
✓ Adopt a Mediterranean-style dietary pattern emphasizing minimally processed foods.
✓ Reduce sugar-sweetened beverages and highly processed foods.
✓ Discuss an appropriate physical-activity program. A combination of cardiovascular and resistance exercise may be beneficial.
✓ Monitor and appropriately manage blood pressure, cholesterol, blood glucose, diabetes, and other cardiovascular risk factors.
✓ Review medications and supplements with your healthcare team.
6. Practical Action Plan for Cirrhosis
If you have compensated or decompensated cirrhosis, nutritional priorities may be very different from those used for uncomplicated fatty liver disease.
Discuss the following with your hepatologist and registered dietitian:
✓ Avoid unnecessary aggressive calorie restriction.
✓ Determine your individualized calorie and protein requirements.
✓ Discuss whether approximately 1.2–1.5 g protein/kg/day is appropriate for your situation.
✓ Avoid prolonged fasting.
✓ Consider smaller, more frequent meals.
✓ Discuss incorporating a carbohydrate- and protein-containing late-evening snack.
✓ If ascites is present, discuss the appropriate sodium target with your healthcare team.
✓ Do not make dietary restrictions so severe that overall food intake becomes inadequate.
✓ Review all prescription medications, over-the-counter medications, herbal products, and dietary supplements with your medical team.
✓ Maintain recommended medical surveillance, including appropriate screening for hepatocellular carcinoma when indicated.

Are my blood pressure, cholesterol, blood glucose, and other cardiovascular risk factors adequately controlled?
Are any of my prescription medications, over-the-counter medications, or supplements potentially harmful to my liver?
If I have cirrhosis, what surveillance do I need for hepatocellular carcinoma and other complications?
8. Questions to Ask Your Registered Dietitian
What should my individualized calorie intake be?
How much protein should I consume each day?
Should I be trying to lose weight, maintain my current weight, or prevent further weight loss?
How can I follow a Mediterranean-style eating pattern while meeting my individual nutritional needs?
How should I distribute food and protein throughout the day?
If I have cirrhosis, what should I eat as a late-evening snack?
If I have ascites, how much sodium should I consume?
How can I reduce sodium without reducing my overall food intake or appetite?
Am I showing signs of malnutrition or muscle loss?
Do laboratory findings indicate that I need any specific vitamin or mineral supplementation?
9. The Patient’s Treatment Pathway
A practical approach to chronic liver disease can be summarized as:
Identify the liver disease → Determine disease severity and fibrosis risk → Identify metabolic and medical drivers → Establish individualized nutrition goals → Treat associated metabolic conditions → Use medication when clinically indicated → Protect muscle and nutritional status → Monitor disease progression and complications → Adjust treatment according to objective results.
The appropriate strategy depends heavily on disease stage. A weight-loss strategy that may be beneficial for a patient with uncomplicated MASLD can be inappropriate for a patient with advanced cirrhosis and sarcopenia.

Chronic liver disease requires stage-specific treatment.
For people with MASLD or MASH and overweight or obesity, improving dietary quality, increasing appropriate physical activity, managing cardiometabolic risk factors, and achieving clinically meaningful weight loss can improve liver health.
